2.) Have you been active for at least 1 month?
Yes, it has been exactly 1 month since I first joined the server.

3.) Have you been helpful to other people on the server? If yes, who and how?
Yes, I've tried to be as helpful as possible by answering questions other players have. I helped jashaungrewal with his town, and was recently accepted as an assistant for polsonator2.

4.) Have you been jailed/muted/rules restricted before?
No.

5.) Do you vote regularly?
I vote at least once a week or more.

6.) Are you going to be active and keep in touch on the forums?
I will be on the server as often as I can to help other players and play with friends. I've made an account on the forum, and always look for new posts and updates.
